MINOR PROTECTION STANDARDS

STANDARDS IMPLEMENTED BY HOTEL KOTARZ

Due to SOM standards requirements, please bring documents proving the identity of the adult legal guardian and the minor (under 18 years old) traveling with you to Hotel Kotarz. If you travel with a child who is not your child or legal ward, please prepare parents' consent for traveling with the child.

REQUIRED DOCUMENTS FOR CHECK-IN

For proper verification, at check-in you will be asked to show a document proving the child’s identity or another document confirming your right to care for the child at that time.

Examples of documents confirming the minor's identity include: ID card, school ID, MObywatel app, Internet Patient Account, court ruling.

If you are not the minor’s parents, presenting a notarized consent or consent from the parents including the child's details, address, parents' phone number, ID number, and the PESEL of the person traveling with the childthe PESEL of the person traveling with the child will be appreciated.
